Graduate Procedures (Non-Thesis, Plan C)

Complete the following steps to earn this degree. It is the student’s responsibility to see that all items are completed.
- Complete an on-line application (Click here for link to on-line application)
- Two official copies of all higher education transcripts
- Application fee (US $55 for all applicants; non-refundable; make checks payable to USU Graduate School)
- Three Letters of Recommendation Two recommendations must be from professors if the applicant has been in school in the last five years; letters are waived for graduates of the MIS Department.
- GMAT Test Results (use institution code 4857)
- TOEFL Test Results (required if English is a second language; use institution code 4857)
The application deadline is June 1 for fall admission, Oct. 1 for spring admission, and March 1 for summer admission. Students with an application in progress, however, may register for up to 12 graduate credits before matriculation.

Matriculation Requirements (matriculation means full, unconditional admission)
- Earned bachelor’s degree
- GPA of 3.2 or higher in the most recent 60 semester credits (or 90 quarter credits) completed
- Minimum GMAT verbal, quantitative, and total scores all in the 40th percentile or higher
- Minimum TOEFL score of 600 on the paper version or 250 on the computer version (from applicants whose first language is not English)
- Favorable letters of reference from legitimate sources
- Permanent U.S. residents may be admitted provisionally under special circumstances. No more than 12 graduate credits may be completed, however, before full matriculation. Provisional matriculation is generally unavailable to international students because of student visa regulations from the U.S. Immigration and Naturalization Service.
Complete Coursework Requirements
- Student must register for at least three credits every semester (except summer) or pay the Graduate School a $15 per semester continuing registration fee.
- Committee formulation (all members must be approved USU graduate faculty):
- Committee chair (also called advisor or major professor)
- MIS committee member
- Committee member outside the MIS Department
- Advisor sends committee recommendation to department head no later than second semester of matriculation.
- Department head completes committee appointment form and sends to Graduate School.
- Graduate School approves committee.
- Advisor and student complete Program of Study for Master’s Degree form (required before the end of the second semester in the program) after committee appointment. MS prerequisites must be listed even if they have been completed already. Prerequisites are to be completed before completing 12 graduate credits.
- Student gets signatures of committee members on form and submits form to department head for approval. (Off-campus students work with their advisor to get signatures.)
- Department head sends two copies of form to Graduate School, keeps one copy in the student’s file and sends one copy to advisor.
- Complete Graduation Requirements (Last semester of course work)
- Student registers for at least 1 credit during the semester in which he or she files graduation application.
- Student submits the following forms to the MIS department head:
- Purple "Completion of Requirements" form signed by advisor (advisor has forms for off-campus students)
- Department MS Evaluation Form signed by student
- Department head sends a copy of purple completion form and department letter of completion to:
- Graduate School
- Student’s MS file
- Student receives graduation papers from Graduate School in the (snail) mail.
- Student completes graduation papers and returns them to the Graduate School.
- Graduate School notifies student, department head, and advisor if there are incomplete items.
- Student graduates!
